There are two application forms one needs to fill while applying for a passport. Form No. 1 is used in case of fresh passport or reissue/replacement of lost or damaged passports. This form is also used for change of name or appearance or exhaustion of pages. Even while applying for passports for minors, form no 1 is used. There is also Personal Particulars Form (PP Form), part of Form No 1 is used for police verification report, and this is often filled separately.
Application Form No. 2 is used in case of applying for police clearance certificate or for change of address or inclusion of spouse’s name or for deletion of ECR stamp.
These forms are available on www.passport.nic.in and applicants can directly download it. There are almost 20 columns to be filled up by the applicants. Candidates are also required to submit 3 photographs of 3.5 cm * 3.5 cm.
Different columns need to be filled with information like;
• Full name with proper documentations,
• Sex,
• Date of birth in dd/mm/yyyy format with birth certificate issued by government or municipal authorities,
• Place and country of birth,
• Name of mother, father and spouse,
• Residential address with phone number,
• Educational qualifications,
• Height in centimeters,
• Citizenship information,
• Minor child and passport information,
• And self declaration.
There is also a certain fee structure to be paid.
Fee structure for passport application is as follows :
• New passport with 10 years validity (36 pages) - Rs 1,000/-
• New passport with 10 years validity (60 pages) – Rs 1,500/-
• Duplicate passport in lieu of damaged, lost, or stolen passport (36 pages) – Rs 2500/-
• Duplicate passport in lieu of damaged, lost, or stolen passport (60 pages) – Rs 3000/-
• New passport for Minors (below 18 years of Age) of 5 years validity or till the minor attains the age of 18 which ever is earlier - Rs 600/-
• In case of change of name, address, date of Birth, appearance, place of Birth, spouse name, name of parents or legal guardian- Rs 1000/-
• For police clearance certificate or ECNR or additional Endorsements- Rs.300/-
